ADOPTION APPLICATION

Are all adults in the home agreement on purchase of a puppy?
Does anyone in your home have allergies to dogs?
How will the puppy be trained?
Are you willing to crate train your dog?
How many hours will your puppy be left alone (without a potty break) each day?
Is this puppy being purchased as a surprise or gift for anyone?
Have you or anyone in the puppy's future household ever been arrested, charged or convicted for animal cruelty or animal hoarding?
Will you be able to take this puppy home on the agreed upon date?
Will you be able to pick up your puppy in Columbus, Ohio?
Do you agree to contact us in the case that you need to re-home this puppy for whatever reason?
Do you agree to spay/neuter your puppy when your veterinarian advises it is time?

APPROVAL AND DEPOSIT:

Applications will be reviewed in the order they are received. If your home is a good fit, you will be notified.

GOING HOME:

You are required to pick up your puppy on the day assigned for their litter. We do offer some flexibility, but this must be discussed prior to pickup day. We charge a weekly boarding fee if your puppy is not picked up before 9 weeks of age. Developmentally the period of 8 weeks to 6 months is very important for bonding and we don’t want our families to miss this important window.

REGISTRATION PAPERS:

With the exception of our AKC poodles, our excellent family companions do not come with registration papers to any registry. The parents of our puppies are registered through AKC, depending on the breed.
